The purpose of this survey is to gather feedback regarding your experience implementing the BETTER program in your organization or practice in collaboration with the BETTER Institute. Your responses will be used to help evaluate our program and the BETTER Institute’s activities. No personally- identifying information will be used or released as part of our evaluation.
