BETTER Health Survey

The BETTER Health Survey is available in Comprehensive, Brief and Lifestyle formats. The surveys are used to capture patient health information helpful in assessing a patient’s cancer and chronic disease risk.

The Brief version is intended to be used for follow-up prevention visits when only changes or updates to a patient’s health status need to be captured.

The Lifestyle version only includes questions and validated tools to assess patients’ current lifestyle risk status. All questions in this version of the health survey are included in the comprehensive and brief versions.

The Primary Prevention and Screening Care Maps provide evidence-informed cancer and chronic disease prevention and screening care paths.

Primary Prevention and Screening Bubble Diagram

The Bubble Diagram is a patient and clinician-friendly representation of the Primary Prevention and Screening Care Map with sex-specific targets for patients at average risk. The blank version allows clinicians to present patients with personalized, key messages about their health.

The Prevention Prescription

The Prevention Prescription summarizes the patients’ primary prevention and screening status.

BETTER Health Survey & Screening Test Guide

The BETTER Health Survey & Screening Test Guide is a tool that can be used to describe chronic conditions and screening tests to patients. It is designed to work in conjunction with the comprehensive version of the health survey.

Cancer Surveillance Tools

Cancer Surveillance Health Survey

The Cancer Surveillance Health Survey is used to capture patient health information, including family history and detailed cancer history, for patients with a personal history of cancer.

Cancer Surveillance Care Map

The Cancer Surveillance Care Map provides evidence-informed cancer surveillance care paths for patients with a personal history of breast, colorectal and/or prostate cancer.

The Cancer Surveillance Bubble Diagram

The Cancer Surveillance Bubble Diagram is a patient and clinician-friendly representation of the Cancer Surveillance Care Map with sex-specific targets. The blank version can be used by clinicians as an agenda-setting tool to present patients with personalized, key messages about their cancer surveillance status.

Cancer Surveillance Prescription

The Cancer Surveillance Prescription is a summary of the patient’s prostate, breast and/or colorectal cancer surveillance status.

First Nations, Indigenous & Métis (FNIM) Tools

BETTER First Nations, Indigenous & Métis (FNIM) Health Survey

The BETTER First Nations, Indigenous & Métis (FNIM) Health Survey is used to capture patient health information for Indigenous populations. The Traditional Medicine & Practices Module captures holistic elements of Indigenous health and wellness that are not captured in the FNIM Health Survey to help inform the care needs of the patient.

The Health & Wellness Wheel

Designed for Indigenous populations, the Health & Wellness Wheel is a patient and clinician-friendly representation of the Primary Prevention and Screening Care Map with sex-specific targets for patients at average risk. The blank version is a tool that allows clinicians to present patients with personalized, key messages about their health.

The BETTER First Nations, Inuit & Métis (FNIM) Health Survey & Screening Test Guide is a tool that can be used to describe chronic conditions and screening tests and is designed to work in conjunction with the BETTER FNIM Health Survey.

Additional Tools

The Goals Sheet helps patients set personalized S.M.A.R.T. (specific, measurable, attainable, realistic, and time-bound) goals to build confidence and make impactful changes in their health. It is designed to work with the Primary Prevention, Cancer Surveillance, and First Nations, Inuit & Métis tools.
