La pr\u00e9valence des maladies chroniques est en constante augmentation, ce qui entra\u00eene des probl\u00e8mes de sant\u00e9, des co\u00fbts de sant\u00e9 \u00e9lev\u00e9s et des d\u00e9c\u00e8s pr\u00e9matur\u00e9s. Il s’agit d’un probl\u00e8me mondial. L’Organisation mondiale de la sant\u00e9 estime que 35 millions de d\u00e9c\u00e8s sont attribuables aux maladies chroniques chaque ann\u00e9e, et ce nombre ne cesse d’augmenter.<\/p>\n\n

La pr\u00e9vention et le d\u00e9pistage des maladies chroniques avant que celles-ci n’explosent demeure le meilleur moyen de freiner cette augmentation, les soins primaires offrant les meilleures possibilit\u00e9s d’am\u00e9liorer la situation \u00e9tant donn\u00e9 que la plupart des actions de pr\u00e9vention et de d\u00e9pistage sont offertes dans ces contextes. Cependant, dans notre syst\u00e8me de sant\u00e9 actuel, la plupart des prestataires de soins primaires manquent de temps, de ressources et d’outils pour aborder la pr\u00e9vention et le d\u00e9pistage du cancer et des maladies chroniques, et pour int\u00e9grer les nombreuses directives qui sont g\u00e9n\u00e9ralement centr\u00e9es sur une maladie, un syst\u00e8me d’organe ou un facteur de risque sp\u00e9cifique.<\/h4>\n\n

Les mod\u00e8les traditionnels de soins de sant\u00e9 sont centr\u00e9s sur une seule maladie, mais pour pr\u00e9venir les maladies chroniques, il est n\u00e9cessaire d’avoir une approche globale et proactive, et c’est l\u00e0 qu’intervient le Programme BETTER\u2122. Nous adoptons une approche personnalis\u00e9e et globale, en int\u00e9grant les meilleures cliniques existantes pour informer les patients sur leurs risques de maladies chroniques et sur les fa\u00e7ons d\u2019am\u00e9liorer leur \u00e9tat de sant\u00e9 global. L\u2019approche BETTER est unique car elle s’attaque aux causes premi\u00e8res des maladies chroniques et aux facteurs de risque communs pour les maladies cardiovasculaires, le diab\u00e8te et le cancer.<\/p>\n\n

Nous r\u00e9unissons des m\u00e9decins, des preneurs de d\u00e9cisions et des responsables de politiques de sant\u00e9 et des chercheurs qui collaborent pour am\u00e9liorer la pr\u00e9vention des maladies chroniques. Les professionnels de soins de sant\u00e9 de premi\u00e8re ligne tels que les infirmi\u00e8res, les nutritionnistes et les pharmaciens sont inform\u00e9s par l’approche et les outils BETTER pour devenir des experts en pr\u00e9vention et une ressource pour leur milieu de pratique.<\/p>\n\n

Bien que les \u00e9tudes aient d\u00e9montr\u00e9 un impact positif du Programme BETTER\u2122 sur la sant\u00e9 des patients \u00e0 travers un gr\u00e2ce \u00e0 un essai contr\u00f4l\u00e9 randomis\u00e9 par groupes, qui a identifi\u00e9 une am\u00e9lioration de 37% du d\u00e9pistage et de la pr\u00e9vention des maladies chroniques telles que la pression art\u00e9rielle, le cancer, le tabagisme, la consommation d’alcool, le r\u00e9gime alimentaire et l\u2019activit\u00e9 physique, de nouvelles recherches portant sur le programme sont actuellement en cours afin de continuer \u00e0 recueillir des donn\u00e9es aupr\u00e8s d\u2019autres populations de patients (par ex. populations vuln\u00e9rables, survivants du cancer), milieux de soins de sant\u00e9 (par ex. sant\u00e9 publique) et conditions de sant\u00e9 (par ex. pr\u00e9vention secondaire des maladies chroniques).<\/p>\n<\/div>\n\n\n\n

BETTER for All est un programme de coaching par les pairs, fond\u00e9 sur des donn\u00e9es probantes, qui favorise un changement de comportement sain afin de r\u00e9duire le risque de cancer et d’autres maladies chroniques dans les \u00e9tablissements de soins de sant\u00e9 communautaires.
