The Prevention Practitioner

The BETTER approach to chronic disease prevention and screening (CDPS) transforms clinical practice through internal practice facilitation, which involves a patient-level intervention conducted by a healthcare professional taken from within the primary care practice setting, the Prevention Practitioner (PP).

Each primary care practice setting identifies one or more personnel available to the group to undertake the specialized role of Prevention Practitioner.  This individual can be a licensed practical nurse, nurse, nurse practitioner, dietician, or other related health professional.  To take on the role of Prevention Practitioner, the individual chosen acquires advanced knowledge of the high-level evidence-based prevention and screening maneuvers available in primary care and which are used in BETTER.

The Prevention Practitioner receives training on the BETTER approach to chronic disease prevention and screening (CDPS) including:

  • Identifying at-risk populations to target for CDPS
  • Determining the appropriate care-path for each CDPS maneuver a patient is eligible to receive
  • Identifying clinical tools available to aid CDPS efforts
  • Adapting and developing tools unique to their practice setting
  • Identifying local, regional and national prevention and screening resources that can be integrated into the practice setting
  • Shared decision-making
  • Motivational interviewing
  • Facilitating communication within the practice setting

The Prevention Practitioner works directly with patients, determines which CDPS maneuvers each patient is eligible to receive and through a process involving shared decision-making and motivational interviewing, develops a unique, personalized “Prevention Prescription” with the patient.
